Coastal Allied Health is able to service clients who hold self-managed & plan-managed NDIS funds.

Hourly Rate of $193.99 + travel - MMM1-5 areas.

Fees are in accordance with the NDIS price guide.

The duration of the appointment is dependent on the support and input required.

Coastal Allied Health is able to service clients who are on a HCP & CHSP recipients.

The duration of the appointment is dependent on the support and input required.

FAQs

First we will require a referral from your HCP Coordinator or NDIS Support Coordinator/Plan Manager or for NDIS self-manged and private clients you can self-refer using our referral form.
From here, our Occupational Therapist, Lucy, will make contact to further discuss your goals and specific needs, to tailor an assessment and/or therapy plan as required.
To be eligible for a Home Care Package you have to meet the following criteria:
* Be an Australian citizen.
* Be over the age of 65.
* Be experiencing a level of difficulty with personal or instrumental activities of daily living.
If you meet the criteria, you are eligible to be assessed for a HCP through My Aged Care. Please contact My Aged Care on 1800 200 422 to arrange an Aged Care Assessment, which will determine the level of care you are eligible for.

To be eligible for the NDIS you have to meet the following criteria:
* Be between 9-65 years of age.
* Be an Australian Citizen.
* Have a disability caused by a permanent impairment.
* Require disability specific support to complete activities of daily life.
